All enrollment forms for Holy Family Students are linked below as fillable PDF documents. Please download each to your computer, type in at your convenience over the summer, print off one of each form for each child, and bring with you to enrollment day!
Paperwork required for ALL students in Preschool – 6th grade:
(please download, fill out and print one PER child — each student needs their own!)
Health Policy Signature Form (one per family) Health Policy in full (please read)
Medical Information Form – Salina Diocese
Parental Medical Consent & Liability Waiver – Salina Diocese
School Directory Form (one per family)
Responsibility Entry Procedure (needed for KP – 2nd graders only)
Change of Info Form (needed only if you have a change in address, phone, employer, e-mail, student health, etc. since last August)
Paperwork required for our NEW students or PRESCHOOL students only:
(our incoming Preschool parents would have already received these in their Preschool packets, but they are available here)
Kansas Dept. of Health Medical Record Packet (wellness packet-must be signed by your physician)
Statement of Immunizations Form
*For incoming preschoolers, be sure to return any additional paperwork from your preschool packet sent out at preschool parent info night!
*New families & incoming preschoolers, also please don’t forget:
- A copy of your child’s birth certificate
- A copy of your child’s baptismal certificate
- A copy of your child’s most recent immunizations from their physician
Paperwork AS NEEDED:
(various dismissal forms, HFE Bucks forms, etc.)
Free & Reduced Meals Application (one per family if you’d like to apply, you must have your own copy per school, we cannot share an application with TMP!)
Student Medication Form (if your child will ever need Tylenol/Advil for headaches/tooth pain/general aches, takes any daily medication, or requires an epi pen/inhaler, please print and complete one form for each medication)
Walking Dismissal Permission Form
After School Care Form (only needed if your child will be attending After School Care — you will also need to stop at the ASC table at enrollment to sign up for the Brightwheel app!)
HFE Bucks Family Agreement Form (one per NEW family if you plan to purchase HFE Bucks)
HFE Bucks Delivery Options Form (one per NEW family if you plan to purchase HFE Bucks)

Lunch fees are determined mid-summer by the State of Kansas and will be emailed out to all parents. A Free & Reduced Meals application will be included in every family enrollment folder, and also posted on our school website and weekly newsletters once we receive this.
If you have any questions, please contact the school office at 785-625-3131 or by e-mail! The school office will be open Tuesdays – Thursdays from 9:00 – 12:00 and 1:00 – 4:00 in June and July.
